PURPOSE/ To assist people of all disabilities to live, work, and play in the community
SERVICES: by setting up support systems that give control back to the consumer. Our teams consist of case managers, transitional living specialists, personal care attendants, and therapists who work together collectively to best serve consumers in their homes.
ELIGIBILITY: We can personalize services for any individual who could benefit from
in-home services, including veterans. Funding possibilities include the Medicaid Home and Community Based Services (HCBS) Traumatic Brain Injury Waiver (TBI) and the Physical Disability Waiver (PD). We also work with the Frail Elderly (FE) Waiver.
AREA SERVED: Greater Kansas City on both sides of the state line
(approximately 90-mile radius)
FEES: HCBS Waiver, PD Waiver, FE Waiver, fee-for-service contract with Kansas Rehabilitation Services. Other payment arrangements can be made as needed and include Workers Compensation and private insurance.
HOURS: 8 a.m. - 4 p.m. Monday - Friday
