PURPOSE/
SERVICES: To assist people with disabilities to live, work, and play in the community by setting up support systems that give control back to the consumer. Our teams consist of case managers, transitional living specialists, personal care attendants and therapists that work together collectively to best serve consumers in their homes.
ELIGIBILITY: Provide services to the Medicaid Home and Community Based Services (HCBS) and the Physical Disability Waiver (PD). We also work with the Frail and Elderly Waiver, Workers Compensation, Kansas Rehabilitation Services, and private insurance.
AREA SERVED: Greater Kansas City area on both sides of the state line (approximately 90 mile radius)
FEES: HCBS Waiver, PD Waiver, FE Waiver, fee-for-service contract with Kansas Rehabilitation Services. Other payment arrangements can be made as needed
HOURS: 8 a.m. to 4 p.m., Monday through Friday
