PURPOSE/

SERVICES:               Promotes functional skills by providing or supporting needed specialty health care to Kansans under the age of 21 who have a disability or chronic disease. Services include diagnostic treatment and outreach clinics.

 

ELIGIBILITY:          Kansas resident under age 21 with covered condition. Family must meet financial guidelines. Each case is considered individually.

 

AREA SERVED:      State of Kansas. This office primarily serves Johnson and Wyandotte counties.

 

FEES:                         None

 

HOURS:                     8 a.m. - 4:30 p.m. Monday - Friday
