CARE stands for Client Assessment, Referral and Evaluation. Everyone seeking a permanent move to a Kansas nursing facility must be assessed by the CARE program.

Frequent Questions

What is a CARE Assessment?

The assessment is a personal interview with the individual and family members or caregivers conducted by hospital personnel or staff from the Area Agency on Aging. The interview questions seek information about the need for assistance with activities such as bathing, meal preparation, housekeeping and laundry.

What happens after the assessment?

The results of the assessment are used to plan for long-term care services. The individual is provided with a certificate that shows the assessment was completed. The individual is also provided with information about community-based alternatives to living in a nursing facility.
